Am I Eligible for Medicare Claims?

You will need to consult your GP, who will then complete a Chronic Disease Management (CDM) form for you to bring to your chiropractic appointment. What is a Chronic Condition?
A chronic condition is defined as a condition that has been present for six months or longer. This may include but is not limited to:
